This website is operated by Start Wellbeing.  START Wellbeing is commissioned by West Sussex County Council Public Health – Starting Well to deliver Healthy Weight for Families service which is a Tier 2 Weight Management Service for Children covering West Sussex.

START Wellbeing do not collect personal information directly through this website (such as forms, cookies, or analytics).

If you choose to make a referral by email, START Wellbeing will receive the personal information you provide:

• Name

• Age

• Gender

• Ethnicity

• No. of siblings

• Address/Postcode

• Telephone number (mobile, home, work)

• Email address

• Medical history and current medical state

• School of child/children

• Any other services involved (E.g. Early Help, Social Services)

 

How information is processed

START Wellbeing will only use the information to:

• Respond to your enquiry.

• Provide our services.

• Provide anonymous data for service monitoring to West Sussex Public Health who commission the service.

START Wellbeing will not sell or share your information with third parties for marketing purposes.

START Wellbeing process personal information under the lawful basis of legitimate interests (responding to enquiries) and, where applicable, legal obligations under the work with commissioners.
